Mission

Riverside Haven Care Home is a licensed Adult Foster Care (AFC) residence located in Portage, Michigan. We provide a safe, supportive, and home‑like environment for adults who need supervision, protection, and assistance with daily living. Our mission is to ensure every resident feels valued, respected, and truly at home.

Population Served

  • Licensed to serve up to six (6) residents.

  • We welcome:

    • Adults age 18 and older with Developmental disabilities

    • Aged

Comprehensive Care Services

Program Goals

  1. Promote independence and dignity for every resident.

  2. Provide individualized care and supervision in a safe, comfortable, home‑like setting.

  3. Support each resident’s physical, emotional, and social well‑being.

  4. Maintain full compliance with state licensing rules and uphold all resident rights.

Personalized Support

  • Room & Board: Furnished bedrooms, nutritious meals, snacks, and housekeeping.

  • Personal Care: Assistance with bathing, dressing, grooming, toileting, and mobility.

  • Supervision & Protection: Staff available 24 hours a day to ensure safety and support.

  • Medication: Administered and supervised in accordance with AFC regulations.

  • Health Monitoring: Observation, documentation, and coordination with physicians and healthcare providers.

  • Social & Recreational Activities: Opportunities for community engagement, group activities, and individualized recreation.

  • Transportation: Arranged transportation for medical appointments and community outings.

Our Facility

Riverside Haven offers a warm, spacious, and thoughtfully designed environment, including:

  • Six private bedrooms, each furnished with a comfortable bed, beddings, closet, mirror, chair, dresser, and a smart flat‑screen TV.

  • Large main‑level living room with a smart TV, opening to a spacious deck with beautiful scenic views.

  • Two dining areas:

    • Main‑level dining room seating up to 8, with a smart TV

    • Lower‑level dining room seating up to 6, adjacent to an activity area

  • Modern kitchen with a small dining area and access to a second deck overlooking serene views.

  • Six bathrooms:

    • Three full bathrooms, each with an open shower and designed to accommodate a shower chair

    • Two half bathrooms

  • Additional amenities:

    • One rest area

    • A computer center

    • High‑speed internet with whole‑house Wi‑Fi coverage
